Instructions

Complete this report under any of the following situations:

1. A congregant or visitor falls ill or receives an injury that requires First Aid or medical treatment while on the church grounds.
2. A congregant or visitor is transported by ambulance from church grounds.
3. An unusual or unexpected incident occurs that jeopardizes the safety of staff, congregants, volunteers or any visitor to the church, there is a vehicle accident (with or without injuries), or above persons are is exposed to a threatening person or situation;
4. An unusual or unexpected incident occurs where unwanted guests or trespasser’s are asked to leave the church grounds.
